| 内容記述 | In adults, bronchial asthma is frequently overlooked or misdiagnosed. Adult-onset bronchial asthma is associated with less stationary disease, which appears to be less stable, more recurrent, and less frequently in remission in comparison to childhood-onset disease. The main risk factors for adult-onset bronchial asthma include obesity, environmental contaminants, and psychological stress. Oxidative stress is an important cause of the acute exacerbation of airway inflammatory diseases, including bronchial asthma. We report the case of a male fisherman in his 40s who experienced incomplete amputation of his left upper arm on a boat. He was emergently transported to hospital. The initial diagnosis included collapse syndrome, rhabdomyolysis, circulatory hypovolemic shock, acute renal failure, electrolyte abnormality and disseminated intravascular coagulation (DIC). After arriving at the hospital, his left upper limb was amputated; however, his experienced cardiopulmonary arrest due to fatal arrhythmia and died 37 hours later. However, unexpectedly, the bronchial lumen was found to be filled with a large amount of mucus with eosinophil infiltration. Goblet cell metaplasia, smooth muscle hypertrophy, and bronchial glandular hyperplasia were obvious, representative of the chronic pathological images of bronchial asthma. Because the patient had no previous medical history, it may be a phenotype of bronchial asthma characterized by adult onset, eosinophilia, possibly non-atopic presentation, and obesity. In this case, due to the deterioration of collapse syndrome, a sharp increase in oxidative stress load may have been the trigger of asthma attack. | |||||||||||||||||||||||||||||
