| 内容記述 | Olfactory dysfunction has attracted increasing attention, particularly during the COVID-19 pandemic. Although subjective olfactory tests are widely used, objective methods for evaluating the olfactory function are limited. Structural imaging modalities such as computed tomography and magnetic resonance imaging are indispensable for diagnosing anatomical abnormalities and the central causes of olfactory disorders. Recently, thallium-based olfactory imaging—also referred to as “olfactory scintigraphy”—has emerged as a promising objective tool for functional assessment. This technique utilizes intranasal thallium-201 (201Tl) administration for visualizing and quantifying tracer migration to the olfactory bulb. Clinical studies have demonstrated its utility in identifying sensorineural olfactory dysfunction associated with various etiologies, including post-viral infections, COVID-19, and traumatic brain injury. This method has also shown potential for predicting treatment outcomes and diagnosing parosmia, a condition that is difficult to assess using conventional tests. The development of this technique was driven by preclinical studies in mice in which 201Tl reliably migrated from the nasal cavity to the olfactory bulb. Unlike other experimental tracers, 201Tl remains the only clinically applicable nuclear imaging agent for olfactory nerve evaluation. Mechanistic studies suggest that altered synaptic dynamics and impaired axonal projections in the olfactory bulb contribute to the pathophysiology of parosmia. Thallium-based olfactory imaging offers a novel, non-invasive approach for visualizing olfactory nerve function and may complement existing diagnostic strategies. Further international validation and regulatory developments are required to expand its global clinical use. |
