In patients presenting with hip joint disorders and groin pain as the chief complaint, clinicians should suspect sacroiliac joint (SIJ) dysfunction and perform a thorough examination, especially when low back pain is reported around the posterior superior iliac spine (PSIS). The SIJ scoring system serves as a convenient screening tool. When the groin pain decreases by more than 70% after lidocaine injection to the intra-articular or posterior ligament of the SIJ, SIJ dysfunction can be confirmed. Relevant treatment strategies include original pelvic mobilization techniques and lumbopelvic support belts. Patients with acetabular dysplasia (AD) may morphologically be predisposed to overload on the SIJ; therefore, in cases showing PSIS tenderness, clinicians should consider SIJ dysfunction as a cause of groin pain. Posterior SIJ ligament injection or pelvic mobilization may alleviate groin pain in patients with AD or femoroacetabular impingement , making these interventions worthwhile trials. Additionally, clinicians should remain vigilant for the so-called hip–SIJ–spine syndrome following AD-related total hip arthroplasty. |
